{"id":1252,"date":"2017-03-13T15:30:42","date_gmt":"2017-03-13T15:30:42","guid":{"rendered":"http:\/\/stg-blogs.bmj.com\/bmjebmspotlight\/?p=1252"},"modified":"2017-08-21T10:21:38","modified_gmt":"2017-08-21T10:21:38","slug":"primary-care-corner-with-geoffrey-modest-md-syncope-guidelines","status":"publish","type":"post","link":"https:\/\/stg-blogs.bmj.com\/bmjebmspotlight\/2017\/03\/13\/primary-care-corner-with-geoffrey-modest-md-syncope-guidelines\/","title":{"rendered":"Primary Care Corner with Geoffrey Modest MD: Syncope Guidelines"},"content":{"rendered":"<p><strong>By Dr. Geoffrey Modest<\/strong><\/p>\n<p>Finally a guideline on syncope management (See\u00a0<a href=\"http:\/\/www.onlinejacc.org\/content\/early\/2017\/03\/03\/j.jacc.2017.03.003\">http:\/\/www.onlinejacc.org\/content\/early\/2017\/03\/03\/j.jacc.2017.03.003<\/a>\u200b\u00a0\u200b, or\u00a010.1016\/j.jacc.2017.03.003), these from the Am Heart Assn\/Am College of Cardiology.\u00a0Many of us, I\u00a0think, have been relying on suggestions from such sources as UpToDate, which are not rigorously evaluated and\u00a0validated\u00a0by at least a semi-independent association (the American Heart Association, though does have intrinsic potential\u00a0biases for more cardiologic workup and investigation than a truly independent organization such as the National Institutes of Health in the US or National\u00a0Institute for Health and\u00a0Care\u00a0Excellence NICE\u00a0in the\u00a0UK, does take the issue of upfront conflicts of interest more seriously than most other specialty groups, in this case with neither the chair nor vice-chair with any stated conflicts of interest, as well as\u00a08\u00a0of the 15 committee\u00a0members without stated conflicts).<\/p>\n<p>Details:<\/p>\n<ul>\n<li>Syncope is a\u00a0common condition: estimates of prevalence as high as 41%, recurrent syncope in 13.5%. Mayo Clinic report of 19% prevalence in\u00a0selected community residents over 45 years old (mean age of 62, more often in females, 22% vs 15%). Trimodal age distribution: first episode age 20, 60, or 80 yo. Different etiologies by age: older people\u00a0more often associated with cardiac causes\/meds\/comorbidities: esp\u00a0aortic stenosis, renal dysfunction, AV or left bundle-branch blocks, and meds associated with\u00a0orthostatic hypotension. Also syncope\u00a0in 12-15% of those with heart failure. younger people: more likely noncardiac<\/li>\n<li>Initial evaluation:\n<ul>\n<li>History\n<ul>\n<li>Focus on prognosis, diagnosis, reversible or amelioratable factors, comorbidities, medication use, and patient\/family needs<\/li>\n<li>Prognostic\u00a0factors depend on separating neural from cardiac causes (the latter being worse), assessing prodromal symptoms (e.g. the most common cause of syncope\u00a0being\u00a0vasovagal, a reflex syncope that\u00a0has typical features of upright posture, exposure to emotional stress,\u00a0pain; associated\u00a0with diaphorsesis,\u00a0warmth, nausea, pallor), family history, meds and comorbidities<\/li>\n<\/ul>\n<\/li>\n<li>PE\n<ul>\n<li>Especially orthostatic blood pressure and pulse changes, murmurs\/gallops\/rubs, basic neuro exam for focal abnormalities that would lead to a more detailed exam<\/li>\n<\/ul>\n<\/li>\n<li>ECG\n<ul>\n<li>Especially bradyarrhythmias with\u00a0sinus pauses\u00a0or high-grade conduction\u00a0block; ventricular tachyarrhythmias. Or arrhythmogenic substrate (WPW, Brugada, long-QT syndrome, hypertrophic cardiomyopathy, arrhythmogenic right\u00a0ventricular cardiomyopathy ARVC)<\/li>\n<\/ul>\n<\/li>\n<li>Then risk assessment\n<ul>\n<li>Stratify by cause of syncope\/reversibility\u00a0of underlying condition (vasovagal has better prognosis\u00a0than heart failure with preserved ejection fraction, which\u00a0is\u00a0better than advanced cardiomyopathy, which is\u00a0better than acute aortic dissection)<\/li>\n<li>Data not really available on stratifying by\u00a0high vs intermediate\u00a0vs low-risk groups. There are some data on short and long-term risk factors (30 days after ER visit or 12 months later, showing the usual culprits\n<ul>\n<li>Short-term: male, older than 60 yo, no prodrome, prior palpitations, exertional syncope, history of structural heart disease, heart failure, cerebrovasc disease, fam history of sudden cardiac death, persistent bleeding, positive troponins<\/li>\n<li>Long-term: many of same, but added diabetes, high CHADS-2 score, cancer, low GFR<\/li>\n<li>And there are some studies suggesting\u00a0&#8220;risk scores&#8221;, but they have\u00a0different definitions of syncope, outcomes, etc., so not so practical, but typically\u00a0include some of the above:\u00a0age cutoff (45 or 65), abnormal ECG, prodrome, exertional, signs of volume depletion, etc.<\/li>\n<\/ul>\n<\/li>\n<li>Further testing: costly and often ineffective as a general rule and should be guided by the above initial exam. in particular, if the initial exam is not reasonably definitive:\n<ul>\n<li>T<strong>argeted blood testing<\/strong>:\n<ul>\n<li>CBC and lytes have low yield when done routinely, and should be guided by history\/physical\u00a0(moderate recommendation, nonrandomized trials)<\/li>\n<li>Brain natriuretic peptide and troponins, uncertain evidence, though they do comment that BNP &#8220;is elevated in patients whose subsequent cause for syncope is determined to be cardiac&#8221;<\/li>\n<\/ul>\n<\/li>\n<li>If eval suggestive of <strong>cardiovascular abnormalities<\/strong>, consider further cardiac workup (not recommended as routine)\n<ul>\n<li>Transthoracic echo, if structural heart disease suspected, esp valvular disease, HCM, LV dysfunction\u00a0(moderate recommendation, nonrandomized trials)<\/li>\n<li>CT\/MRI: MRI if suspect infiltrative disease\u00a0such as sarcoid, or if suspect ARVC; CT esp\u00a0if\u00a0suspect pulmonary\u00a0embolism\u00a0(weak recommendation, nonrandomized trials)<\/li>\n<li>Stress testing: esp if syncope during exertion (moderate recommendation\u200b, limited design)<\/li>\n<li>Cardiac monitoring: if suspected arrhythmia, choice dependent on likely timing of next syncope (Holter if likely in next 24-72 hours, others if longer). (moderate\u00a0recommendation\u200b, limited design). BUT they do push some for implantable cardiac monitors, which has the highest yield\u00a0for those with no answer from noninvasive testing\u00a0(moderate\u00a0recommendation, with RCTs)<\/li>\n<li>In-hospital monitoring: good to do telemetry<\/li>\n<li>Electrophysiological studies: recommendations\u00a0based on\u00a0older studies. Can be useful in those with syncope from suspected arrhythmia, mostly useful in those with\u00a0structural heart disease (yield of 50% vs 10%).\u00a0(Moderate recommendation, nonrandomized trials)<\/li>\n<li>Tilt-table testing: though they give this a moderate recommendation with RCTs to support, they do note that the utility is highest in patients with suspected recurrent\u00a0vasovagal syncope, BUT there is overall only &#8220;moderate sensitivity, specificity, and\u00a0reproducibility,&#8221;\u00a0with\u00a0&#8220;the presence of false-positives in controls&#8221;, and the diagnosis can typically be made through\u00a0structured history taking as well and\u00a0long-term cardiac monitoring.<\/li>\n<\/ul>\n<\/li>\n<li>If eval suggestive of <strong>neurogenic orthostatic hypotension<\/strong>, also not to be done routinely. Can be associated with multiple system atrophy, Parkinson&#8217;s, Lewy Body dementia, or peripheral autonomic dysfunction from diabetes, amyloidosis, immune-mediated neuropathies, hereditary sensory or autonomic neuropathies, inflammatory neuropathies. Less commonly with B12 deficiency, neurotoxins,\u00a0porphyria, HIV or\u00a0other infections\n<ul>\n<li>EEG during tilt-testing: patients\u00a0can have both epileptic convulsions as well as\u00a0pseudosyncope, which can be sorted out by doing tilt-testing with EEG monitoring\u00a0(moderate\u00a0recommendation\u200b, limited design)<\/li>\n<li>Head\u00a0MRI and CT: no benefit in absence of focal neuro deficits or head injury<\/li>\n<li>Carotid artery imaging: no benefit without focal neuro deficits<\/li>\n<li>Plain EEG: no benefit unless symptoms suggestive of a seizure<\/li>\n<\/ul>\n<\/li>\n<li>Then treatment decision\n<ul>\n<li>There is a large section on guideline-based therapies for arrhythmias and structuralcardiac disorders, which i will not review here<\/li>\n<li>Vasovagal syncope: patient education on diagnosis and triggers, lying down when symptoms begin\u00a0(if sufficiently long prodrome), and in recurrent syncope:\n<ul>\n<li>Midodrine if no history of hypertension, heart failure or urinary retention. Studies suggest 43% reduction in symptoms. (moderate recommendation, RCTs)<\/li>\n<li>Orthostatic training:\u00a0e.g. repetitive tilt-table tests, or daily\u00a0standing against a wall in the house for prolonged time periods (weak\u00a0recommendation, RCTs)<\/li>\n<li>Fludrocortisone, esp if inadequate response to fluids and salt\u00a0(weak\u00a0recommendation, nonrandomized trials)<\/li>\n<li>Other approaches with weak recommendations include: b-blockers in those &gt;42 years old (poor performance in younger patients), encouraging\u00a0increased\u00a0salt and fluid intake, decreasing meds that\u00a0cause\u00a0hypotension, SSRIs<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>Commentary:<\/p>\n<ul>\n<li>Though CBC and lytes do have a low yield, I personally will continue to\u00a0do them regularly, since they are cheap\/easy and may unexpectedly\u00a0lead to an important intervention by finding an unexpected anemia or hypokalemia, etc.<\/li>\n<li>I would reinforce the importance of manually checking baseline and orthostatic blood pressure.
