But the Herceptin debate is certainly not over \u2013 the benefit is small, the cardiac dangers great, the long-term effects unknown, the dosage uncertain and the costs enormous \u2013 as pointed out in the accompanying <a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS014067360760004X\/fulltext\">editorial<\/a>.<!--more--><\/p>\n<p>Ever heard of <a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140673607600294\/fulltext\">medium-chain acyl-CoA dehydrogenase deficiency<\/a>? As you\u2019ll have guessed, it\u2019s a rare metabolic disorder &#8211; potentially lethal but treatable. Over a ten-year period, 81 cases were picked up in Australia, and this <a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140673607600294\/fulltext\">paper <\/a>makes an argument for neonatal screening using tandem-mass spectrometry.<\/p>\n<p>Good news about <a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140673607600300\/fulltext\">premature or very low birthweight babies<\/a> \u2013 there has been a notable decline in <a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140673607600300\/fulltext\">cerebral palsy<\/a> rates in European countries between 1980 and 1996.<\/p>\n<p>A lengthy <a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140673607600312\/fulltext\">review <\/a>of <a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140673607600312\/fulltext\">caries<\/a>, containing plenty of cavities filled with an amalgam of knowledge and opinion.<\/p>\n<p>If we could really help our patients <a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140673607600336\/fulltext\">lose weight<\/a> and keep it lost, it would be a major breakthrough. We have three drugs to help our fat patients \u2013 <a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140673607600336\/fulltext\">orlistat<\/a>, <a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140673607600336\/fulltext\">sibutramine <\/a>and <a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140673607600336\/fulltext\">rimonabant<\/a>. As this <a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140673607600336\/fulltext\">review <\/a>makes clear, none is ideal and we don\u2019t have any real evidence about their effects on hard end-points like cardiovascular disease or osteoarthritis.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>This study shows that when given early after adjuvant chemotherapy for HER-2 positive breast cancer, trastuzumab provides significant mortality benefit in as little as two years.
