{"id":40513,"date":"2017-10-31T10:37:32","date_gmt":"2017-10-31T09:37:32","guid":{"rendered":"http:\/\/stg-blogs.bmj.com\/bmj\/?p=40513"},"modified":"2017-11-10T17:43:07","modified_gmt":"2017-11-10T16:43:07","slug":"ceinwen-giles-jeremy-hunt-stole-my-biscuits","status":"publish","type":"post","link":"https:\/\/stg-blogs.bmj.com\/bmj\/2017\/10\/31\/ceinwen-giles-jeremy-hunt-stole-my-biscuits\/","title":{"rendered":"Ceinwen Giles: Jeremy Hunt stole my biscuits"},"content":{"rendered":"<p class=\"standfirst\">Stopping the clinic&#8217;s biscuit supply may seem trivial, but small things make a difference, says Ceinwen Giles<\/p>\n<p><!--more--><a href=\"https:\/\/stg-blogs.bmj.com\/bmj\/files\/2015\/09\/ceinwen_giles2.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-35159\" src=\"https:\/\/stg-blogs.bmj.com\/bmj\/files\/2015\/09\/ceinwen_giles2.jpg\" alt=\"ceinwen_giles2\" width=\"188\" height=\"121\" \/><\/a><\/p>\n<div id=\"contentsContainer\">\n<div id=\"contents\">\n<p id=\"E55\"><span id=\"E56\">\u201cIt\u2019s outrageous.\u201d<\/span><\/p>\n<p id=\"E58\"><span id=\"E59\">\u201cPretty soon there won\u2019t be anything left to cut.\u201d<\/span><\/p>\n<p id=\"E62\"><span id=\"E63\">\u201cWhat the <\/span><span id=\"E65\">actual\u2026.???\u201d<\/span><\/p>\n<p id=\"E68\"><span id=\"E69\">These w<\/span><span id=\"E70\">ere the comments I heard from<\/span><span id=\"E71\"> nurses and<\/span><span id=\"E72\"> fellow patients at <\/span><span id=\"E73\">my <\/span><span id=\"E74\">immunology <\/span><span id=\"E75\">clinic a few months ago. The source of their ire was not, as one might expect, the cutting of access to treatment or extended wait<\/span><span id=\"E76\">ing<\/span><span id=\"E77\"> time<\/span><span id=\"E78\">s\u2014<\/span><span id=\"E79\">it was biscuits. <\/span><\/p>\n<p id=\"E81\"><span id=\"E82\">I\u2019ve been attending the same<\/span><span id=\"E83\"> <\/span><span id=\"E84\">clinic at a large, modern hospital for fi<\/span><span id=\"E85\">ve and a half years. Because <\/span><span id=\"E87\">the<\/span><span id=\"E88\"> treatment <\/span><span id=\"E89\">I receive <\/span><span id=\"E90\">is administered by IV<\/span><span id=\"E92\"> every four weeks, I\u2019ve gotten to know many of the other patients fairly well. We<\/span><span id=\"E93\"> have an odd sort of <\/span><span id=\"E94\">camaraderie<\/span><span id=\"E95\">,<\/span><span id=\"E96\"> sitting together hooked up to various <\/span><span id=\"E97\">bits of equipment<\/span><span id=\"E98\"> and drugs<\/span><span id=\"E99\"> for hours at a time. W<\/span><span id=\"E100\">e chat about our families,<\/span><span id=\"E101\"> holidays, and current events, b<\/span><span id=\"E102\">ut few conversations have become as animated as what I <\/span><span id=\"E103\">now <\/span><span id=\"E104\">think of as \u201c<\/span><span id=\"E106\">Biscuitgate.<\/span><span id=\"E108\">\u201d<\/span><\/p>\n<p id=\"E110\"><span id=\"E112\">Biscuitgate<\/span><span id=\"E114\"> started three months ago when we turned up at our usual day and time to find that the clinic\u2019s biscuit tin was empty. It soon emerged that the tin was empty due <\/span><span id=\"E115\">not <\/span><span id=\"E116\">to oversight, but<\/span><span id=\"E117\"> rather<\/span><span id=\"E118\"> to financial cuts. <\/span><\/p>\n<p id=\"E120\"><span id=\"E122\">\u201cIt started years ago\u201d<\/span><span id=\"E124\">, <\/span><span id=\"E126\">one fellow patient mused<\/span><span id=\"E128\">. \u201cThey cut the <\/span><span id=\"E130\">Branston<\/span><span id=\"E132\"> <\/span><span id=\"E133\">pickle in the cheese and pickle sandwiches, and it\u2019s been downhill from there.\u201d<\/span><\/p>\n<p id=\"E135\"><span id=\"E136\">\u201c<\/span><span id=\"E137\">That <\/span><span id=\"E138\">Jeremy Hunt stole my biscuits\u201d, <\/span><span id=\"E139\">muttered <\/span><span id=\"E140\">another patient <\/span><span id=\"E141\">loudly<\/span><span id=\"E142\">. <\/span><\/p>\n<p id=\"E144\"><span id=\"E145\">Indeed, the last few years have<\/span><span id=\"E146\"> seen an increasingly rapid process of <\/span><span id=\"E147\">cut<\/span><span id=\"E148\">s<\/span><span id=\"E149\"> to the snacks offered to patients. It may have started with<\/span><span id=\"E150\"> the<\/span><span id=\"E153\"> pickle<\/span><span id=\"E154\">,<\/span><span id=\"E155\"> but it<\/span><span id=\"E156\"> <\/span><span id=\"E157\">expanded to <\/span><span id=\"E158\">all <\/span><span id=\"E159\">sandwiches, orange juice, and yogurt. The biscuits and tea were all we had left. <\/span><\/p>\n<p id=\"E161\"><span id=\"E162\">H<\/span><span id=\"E163\">ere\u2019s the thing: I don\u2019t even usually eat these biscuits<\/span><span id=\"E164\">. <\/span><span id=\"E165\">But hav<\/span><span id=\"E166\">ing witnessed the uproar that the de-<\/span><span id=\"E168\">biscuitisation<\/span><span id=\"E170\"> of the clinic caused, I decided I needed to fight for the right of my fellow patients to have their infusions with a side of<\/span><span id=\"E171\"> cookies whether I personally eat<\/span><span id=\"E172\"> them or not.<\/span><span id=\"E173\"> <\/span><\/p>\n<p id=\"E175\"><span id=\"E176\">Online research showed me that a catering box of 100 biscuit<\/span><span id=\"E177\">\u00a0packets<\/span><span id=\"E178\"> cost<\/span><span id=\"E179\">s<\/span><span id=\"E180\"> <\/span><span id=\"E181\">approximately<\/span><span id=\"E182\"> \u00a318<\/span><span id=\"E183\">. Estimates by <\/span><span id=\"E184\">my <\/span><span id=\"E185\">nurses vary<\/span><span id=\"E186\"> but, on average, the clinic gets through two to three boxes a month. At <\/span><span id=\"E187\">this price<\/span><span id=\"E188\"> we\u2019re talking about a cost of<\/span><span id=\"E189\"> <\/span><span id=\"E190\">around <\/span><span id=\"E191\">\u00a313 a week. <\/span><span id=\"E192\">The NHS Trust where I receive my treatment has an annual budget of more than \u00a3800 million<\/span><span id=\"E193\">.<\/span><span style=\"font-size: 1rem\">\u00a0<\/span><\/p>\n<\/div>\n<\/div>\n<div id=\"contentsContainer\">\n<div id=\"contents\">\n<p id=\"E195\"><span id=\"E196\">I initially<\/span><span id=\"E198\"> approached a non-executive d<\/span><span id=\"E199\">irector of the hospital who<\/span><span id=\"E200\">m<\/span><span id=\"E201\"> I know personally, and<\/span><span id=\"E202\"> I told her of our <\/span><span id=\"E203\">pligh<\/span><span id=\"E204\">t. <\/span><span id=\"E205\"> I pointed out that during hours of a somewhat unpleasant treatment, the biscuits are a small kindness. When the treatment makes us tired and nauseous, as it often does, the biscuits offer a small sugar boost and can help settle the stomach. Yes, we can bring food with us (many of us do) but when you\u2019re sitting in the same chair for four hours at a go, the food goes quickly.<\/span><\/p>\n<p id=\"E207\"><span id=\"E208\">The director<\/span><span id=\"E209\"> <\/span><span id=\"E210\">understood how much small things <\/span><span id=\"E211\">can matter<\/span><span id=\"E212\">, and promised to look into it. The biscuits made a <\/span><span id=\"E213\">short-lived <\/span><span id=\"E214\">reappearance<\/span><span id=\"E215\">, <\/span><span id=\"E216\">but <\/span><span id=\"E217\">this month they were cut again. Word on the stree<\/span><span id=\"E218\">t (or hospital corridor, as it may be) is that although the matron has<\/span><span id=\"E219\"> approved the biscuits, an operations<\/span><span id=\"E220\"> manager of some description has<\/span><span id=\"E221\"> re<\/span><span id=\"E222\">fused to sign off the request. In any case, t<\/span><span id=\"E223\">he biscuit tin is again lying empty.<\/span><\/p>\n<p id=\"E225\"><span id=\"E226\">The repeat cutting of the biscuits led me to question whether I was fighting a losing<\/span><span id=\"E227\">,<\/span><span id=\"E228\"> or perhaps unimportant<\/span><span id=\"E229\">,<\/span><span id=\"E230\"> battle. The clinic I attend does, after all, provide me wi<\/span><span id=\"E231\">th very expensive drugs for free. <\/span><span id=\"E232\">However, having now debated t<\/span><span id=\"E233\">his topic<\/span><span id=\"E234\"> far and wide,<\/span><span id=\"E235\"> my conclusion remains<\/span><span id=\"E236\"> the same: the biscuits matter. <\/span><\/p>\n<p id=\"E238\"><span id=\"E239\">We may be living in the age of austerity, but <\/span><span id=\"E240\">good<\/span><span id=\"E241\"> patient experience should be seen a fundamental dimension of healthcare quality, not just a nice-to-have afterthought. With increasing cuts <\/span><span id=\"E242\">and little hope of more money for the NHS anytime soon, <\/span><span id=\"E243\">we all need to make difficult decisions,<\/span><span id=\"E244\"> but surely these should be made in partnership with those who are impacted most? <\/span><\/p>\n<p id=\"E246\"><span id=\"E247\">W<\/span><span id=\"E248\">hoever is responsible for the raid on biscuits has, as far as I can tell, never spoken to the staff or patients in this clinic about their experiences. Instead, they\u2019ve made a unilateral decisio<\/span><span id=\"E249\">n without understanding the true value of the biscuits. <\/span><span id=\"E250\">I\u2019d postulate that <\/span><span id=\"E251\">\u00a313 spent o<\/span><span id=\"E252\">n biscuits is probably more than returned in<\/span><span id=\"E253\"> terms of an increase in trust <\/span><span id=\"E254\">and respect f<\/span><span id=\"E255\">or the hospital and it\u2019s staff, as well as improved adherence to treatment protocols.<\/span><span id=\"E256\"> <\/span><\/p>\n<p id=\"E258\"><span id=\"E259\">I\u2019ve raised another complaint, but have yet to hear if any chang<\/span><span id=\"E260\">e is in the offing. In the meantime, if you see a group of people outside a hospital with <\/span><span id=\"E261\">a sign<\/span><span id=\"E262\"> <\/span><span id=\"E264\">saying<\/span><span id=\"E266\"> \u201cThese cuts really take the biscuit\u201d, you\u2019ll know why.<\/span><\/p>\n<\/div>\n<p><em><strong>Ceinwen Giles<\/strong> is a director at Shine Cancer Support and a trustee of the Point of Care Foundation.
