But there are no mandated regular mental health assessments, apart from screening for alcohol and drug problems,\u201d says Suresh, calling for a revalidation-type system for pilots.<\/p>\n<p><strong><a href=\"https:\/\/stg-blogs.bmj.com\/bmj\/files\/2015\/03\/home_visit.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-thumbnail wp-image-33805\" src=\"https:\/\/stg-blogs.bmj.com\/bmj\/files\/2015\/03\/home_visit-150x150.jpg\" alt=\"home_visit\" width=\"150\" height=\"150\" \/><\/a>Feature: <\/strong><a href=\"http:\/\/www.bmj.com\/content\/350\/bmj.h1456\"><strong>The health coaches from Dunkin\u2019 Donuts<\/strong><\/a><\/p>\n<p>\u201cIt is very countercultural that someone from, say, Dunkin\u2019 Donuts [a fast food outlet] can transform healthcare . . . Our healthcare services have become centred around a series of transactions, when what we need to do is base them around relationships,\u201d says the chief executive of Iora Health, which explores novel models of peer to peer coaching.<\/p>\n<p>Health coaching, broadly defined as \u201chelping patients to help themselves,\u201d is increasingly seen as an answer to the ever pressing question of how to make healthcare more effective without spending more, throughout the United States, and more recently in Europe, <a href=\"http:\/\/www.bmj.com\/content\/350\/bmj.h1456\">writes Sophie Arie in this feature<\/a>.<\/p>\n<p>In many cases, nurses and community health workers have been trained to provide this support, usually by telephone. But the idea of people with no medical expertise working alongside health professionals to get to know the people behind the illness is gaining credence, Arie finds.<\/p>\n<p><strong>Personal View: <a href=\"http:\/\/www.bmj.com\/content\/350\/bmj.h19\">Automated alerts for acute kidney injury warrant caution<br \/>\n<\/a><\/strong><br \/>\n\u201cWith the exception of the small percentage of cases with primary kidney disease, AKI [acute kidney injury] is essentially a marker of severity of illness. . .
