{"id":289,"date":"2007-07-23T08:34:09","date_gmt":"2007-07-23T07:34:09","guid":{"rendered":"http:\/\/resource.bmj.com\/bmj\/2007\/07\/23\/ann-intern-med-17-jul-2007-vol-147\/"},"modified":"2007-07-23T08:34:09","modified_gmt":"2007-07-23T07:34:09","slug":"ann-intern-med-17-jul-2007-vol-147","status":"publish","type":"post","link":"https:\/\/stg-blogs.bmj.com\/bmj\/2007\/07\/23\/ann-intern-med-17-jul-2007-vol-147\/","title":{"rendered":"Ann Intern Med  17 Jul 2007  Vol 147"},"content":{"rendered":"<p>When penicillin was a new drug in short supply, its use in gonorrhea became the subject of heated debate; after that, decades passed before the first penicillin-resistant gonococci emerged and led to the abandonment of penicillin in favour of fluoroquinolone antibiotics.<!--more-->\u00a0Now these in turn are losing their effectiveness, according to <a href=\"http:\/\/www.annals.org\/cgi\/content\/abstract\/147\/2\/81\">a survey of gonococcal resistance in the USA between 1988 and 2003<\/a>. But penicillin sensitivity has returned widely to prevalent gonococci and there are many other antibiotics to which they remain susceptible.<\/p>\n<p>One person in 400 between the ages of 14 and 39 in the USA has gonorrhea, but ten times that number have <a href=\"http:\/\/www.annals.org\/cgi\/content\/abstract\/147\/2\/117\">chlamydia<\/a>. This is a major cause of female infertility and pelvic pain, and they like us, fret about the best screening strategy. The US Preventive Services Task Force takes its evidence-gathering seriously and there are <a href=\"http:\/\/www.annals.org\/cgi\/content\/abstract\/147\/2\/117\">two<\/a> methodology <a href=\"http:\/\/www.annals.org\/cgi\/content\/abstract\/147\/2\/123\">papers<\/a> here\u00a0 plus a <a href=\"http:\/\/www.annals.org\/cgi\/content\/abstract\/147\/2\/135\">summary of their systematic review<\/a> and the <a href=\"http:\/\/www.annals.org\/cgi\/content\/abstract\/147\/2\/128\">recommendations<\/a> themselves which are:<br \/>\n* Screen all sexually active women age 24 or younger<br \/>\n* Screen older pregnant women at increased risk<br \/>\n* Do not screen age 25 or older unless at increased risk<br \/>\n* Not enough evidence to screen men.<br \/>\nBut follow different guidelines in the UK.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>When penicillin was a new drug in short supply, its use in gonorrhea became the subject of heated debate; after that, decades passed before the first penicillin-resistant gonococci emerged and led to the abandonment of penicillin in favour of fluoroquinolone antibiotics.
