March&#8217;s #ADC_JC"},"content":{"rendered":"<p><a href=\"https:\/\/stg-blogs.bmj.com\/adc\/files\/2014\/05\/246-20324-1.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-thumbnail wp-image-924\" src=\"https:\/\/stg-blogs.bmj.com\/adc\/files\/2014\/05\/246-20324-1-150x150.jpg\" alt=\"-246-20324-1\" width=\"150\" height=\"150\" \/><\/a><\/p>\n<p>So &#8211; this blog is the beginning of a rather excellent <a href=\"https:\/\/storify.com\/dralangrayson\/bronchiolitis-the-future\">&#8220;Storify&#8221; summary <\/a>of the March&#8217;s <a title=\"Archives of Disease in Childhood\u2019s Twitter Journal Club \u2013 #ADC_JC\" href=\"https:\/\/stg-blogs.bmj.com\/adc\/twitterclub\/\">#ADC_JC<\/a> which debated an RCT examining the use of heated humidified high-flow nasal cannula (HHHFNC) instead of hypertonic saline (HSS) in the management of bronchiolitis in the emergency department.<\/p>\n<p>(For those who don&#8217;t know, Storify is a lovely way of capturing social media-based events and giving them a context and a thread &#8230; which the moderator of this journal club, @DrAlanGrayson has done beautifully.)<\/p>\n<p>For the real experience, pop over to the <a href=\"https:\/\/storify.com\/dralangrayson\/bronchiolitis-the-future\">full version<\/a>, but whet your appetites here &#8230;<\/p>\n<p>&nbsp;<\/p>\n<div id=\"abstract-1\" class=\"section abstract\">\n<h2>Bronchiolitis. I reckon there&#8217;s plenty of room within <a href=\"http:\/\/lifeinthefastlane.com\/foam\/\">FOAM <\/a>for both.<\/p>\n<h4>Bronchiolitis<\/h4>\n<p>For less-experienced readers, bronchiolitis is a viral illness, classically caused by Respiratory Syncytial Virus, occurring in the under-2 age group and in the winter months. The first &#8220;bronchy&#8221; cough of the year, usually in late September in the UK, is a clarion call to the children&#8217;s doctor and a reminder to order more caffeine &#8211; for the apnoeic kids and for the doc, as it may be a long winter of long shifts&#8230;<\/p>\n<p><a href=\"https:\/\/storify.com\/dralangrayson\/bronchiolitis-the-future\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter\" src=\"https:\/\/i.embed.ly\/1\/display\/resize?key=1e6a1a1efdb011df84894040444cdc60&amp;url=http%3A%2F%2Fwww.medical-artist.com%2Fassets%2Fimages%2FBronchiolitis-poster.jpg&amp;width=490\" alt=\"\" width=\"490\" height=\"314\" \/><\/a><br \/>\nThis beautiful illustration, from the wonderful site medical-artist.com, shows the bronchioles filling with mucus, which leads to difficulty in breathing.<\/p>\n<p><!--more--><br \/>\nMost kids are fine, but a significant proportion will need treatment in the PED, with a smaller admission rate. The authors suggest that, in those admitted, 10% of healthy children and ~36% of those with cardiorespiratory comorbidities will need PICU.<\/p>\n<p>&nbsp;<\/p>\n<p><a href=\"https:\/\/storify.com\/dralangrayson\/bronchiolitis-the-future\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter wp-image-922 size-full\" src=\"https:\/\/stg-blogs.bmj.com\/adc\/files\/2014\/05\/resize.jpg\" alt=\"resize\" width=\"490\" height=\"294\" srcset=\"https:\/\/stg-blogs.bmj.com\/adc\/files\/2014\/05\/resize.jpg 490w, https:\/\/stg-blogs.bmj.com\/adc\/files\/2014\/05\/resize-300x180.jpg 300w\" sizes=\"auto, (max-width: 490px) 100vw, 490px\" \/><\/a><br \/>\nThe chest sounds are distinctive; I was unable to find a sound file of stethoscopy, but this may help.<\/p>\n<p>https:\/\/www.youtube.com\/watch?v=RFwr_zbgJII<\/p>\n<p>Treatment until recently has been supportive, with the sickest kids needing respiratory support, up to the level of intubation and ventilation and nutritional support (NG feeding and IVI).<\/p>\n<p>Hypertonic saline solution (HSS) has been used in my PED for about 5 years. It&#8217;s mechanism is thought to be by osmosis, softening the mucus and allowing ciliary clearance.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter\" src=\"http:\/\/images.alfresco.advanstar.com\/alfresco_images\/HealthCare\/2012\/10\/27\/7aaa6b36-aa36-4625-b487-f78b03e9d490\/Figure-1a-1b-1c-inset_t.jpg\" alt=\"\" width=\"200\" height=\"69\" \/><\/p>\n<p>Sorry the image is small. This <a href=\"http:\/\/contemporarypediatrics.modernmedicine.com\/contemporary-pediatrics\/news\/modernmedicine\/modern-medicine-feature-articles\/bronchiolitis-focus-hyp?page=full\">link redirects to a useful article on HSS. <\/a>Please note that HSS hasn&#8217;t been shown to reduce mortality or PICU admission, merely Length of Stay.<\/p>\n<p>&nbsp;<\/p>\n<p>Bronchiolitis is arguably the most common significant medical illness of childhood. At least 1 in 7 normal infants will develop symptomatic bronchiolitis in his or her first year of life. 1 Hospitalization of infants for bronchiolitis is responsible for an annual expenditure of more than a half billion dollars.<\/p>\n<p>The authors compare this with Hi-flow oxygen, delivering higher rates of humidified O2 than with standard nasal cannulae.
